Transitioning to Adult Care



When should I transition or think about going to an adult doctor?           

Typically, we often like to see our older patients transition into adult care when they have assumed his/her adult role. For example:

*Graduated from College*
*Engaged to be married and/or married"
*In the work force*
*Expecting a child of his/her own*



How do I find another doctor?

*Ask your current provider for a referral or recommendation*
*Look in the phone book under "Physicians" in the Yellow Pages*
*Call your insurance company and ask who is in your "network" for your local area*
*Check with your local support groups*
*Ask a Vocational Rehabilitation Counselor*
*Call large medical and rehabilitation centers*
*Ask friends and family who their providers are*

What do I do after I found a new doctor?
Once you have found a new doctor, you will want to call to make sure they are accepting new patients and book your first appointment. Do not hesitate to ask any questions you may have about the new practice. For example, "Do you happen to accept CIGNA health insurance?"

After you have registered with the new office (given your name, date of birth, mailing address, phone number, insurance information, etc), you will want to "transfer your records" to your new provider.

MEDICAL RECORDS RELEASE

Fill out the top portion of this form. If you are transferring to another doctor's office, you would check off the first option, "Transferring to Another Practice". Please include in the spaces provided the NEW providers name and mailing address.

Sign and date the bottom of the form and send to Plymouth Pediatrics. Our address and fax number is listed at the base of the form. *Note - If you are under 18 years of age, please have your legal guardian sign for you.

Upon receipt of the Transfer of Records form, our office will copy and mail your records to your new provider. (This normally  takes up to 5 business days to complete this transaction.)

Can I be seen at Plymouth Pediatrics before I fully transfer to another office?
Yes. Our providers actually prefer to see you one last time to assist in making the final transition from Pediatrics to Adulthood as easy as possible for you.
